A few important edits before you publish:

  • Replace every bracketed placeholder with your real information. 
  • Remove any sentence that is not true for your business. 
  • If you share leads with another company, say that explicitly. 
  • If you use Google Ads, Meta Pixel, call tracking, or analytics, keep the cookie/tracking section. 
  • If you serve California residents, add the Notice at Collection under the form.  

This is a practical draft, not legal advice. Because privacy compliance depends on your actual lead flow, vendors, tracking tools, and states targeted, a lawyer should review the final version before you rely on it for a regulated financial-services campaign.
